Job summary

Windsor Home Health, a division of Premier Therapy, is seeking a full-time Licensed Practical Nurse (LPN) to join our home health team and help patients remain comfortable at home. You will provide nursing care under the supervision of the RN and collaborate with the plan of care to achieve positive outcomes.

We value compassionate, reliable nurses who communicate well with families and the multidisciplinary team.

Qualifications

• Proof of graduating from an accredited school of practical nursing.
• Current LPN license from the State of Ohio.
• Previous home health care experience preferred.
• Current CPR certification.
• TB test, physical examination and a criminal background check completed.

Responsibilities

• Provide nursing care under the direction of the Home Health RN and the Plan of Care to achieve positive patient outcomes.
• Communicate with family members and the patient's multidisciplinary team.
