Heritage Health Partners is seeking a reliable LPN to care for a child in a professional and considerate manner in the Clarkston/Stone Mountain area. This Full-Time position offers flexible hours and a supportive team.

Formación

• Proven experience as LPN or similar role.
• Ability to undertake a variety of diverse care tasks.
• Understanding of confidentiality obligations and nursing best practices.
• Excellent communication and interpersonal skills.

Responsabilidades

• Triage patient/family calls and determine appropriate interventions.
• Complete assigned visits during office hours.
• Provide patient and/or caregiver with instructions for safe home care.
• Document nursing assessments/interventions and patient contacts in EMR per policy.
• Obtain consent forms and complete needs assessment as required.
• Remain accessible by phone and available during on-call periods.
• Maintain confidentiality of patients and families, co-workers, and organization.
• Adhere to Standards of Conduct and HIPAA regulations.
• Report any misconduct or violations.
