Job summary

CommonSpirit Health at Home is seeking an experienced LPN/LVN to deliver direct patient care in home settings under physician plans. You will collaborate with the RN Case Manager and multidisciplinary team to ensure high-quality care and adherence to guidelines.

The role requires at least one year of nursing experience (home care preferred), CPR/BLS, and strong communication. You will document care, educate patients and families, and participate in case conferences.

Qualifications

• Graduate of an accredited practical nursing program and licensed to practice in the state.
• Current CPR/BLS certification required and must be maintained.
• Minimum of one year of nursing experience; home care or hospital preferred.
• Excellent clinical skills and strong communication abilities.

Responsibilities

• Deliver direct patient care per approved treatment plans.
• Collaborate with RN Case Manager and healthcare team.
• Conduct patient visits per physician plan and comply with regulations.
• Document care accurately and timely.
• Educate patients and caregivers on care plans.
• Maintain patient safety and follow protocols.
