Qualifications

• Current and unrestricted Licensed Practical Nurse licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• CPR certification preferred or ability to complete within 90 days of hire.
• Ability to work flexible hours as required to meet identified client needs.
• Ability to manage multiple tasks simultaneously.
• Able to work independently; good communication, writing, and organizational skills.

Responsibilities

• Provides high quality clinical nursing services within scope of practice and plan of care.
• Documents patient visits thoroughly in the electronic medical record per regulatory and payer requirements.
• Participates in care coordination, reporting patient needs and changes in condition to the team.
• Adapts to diverse home situations regardless of patient demographics.
