Elara Caring is seeking a Licensed Vocational / Practical Nurse to support home health services in Williams County. The role involves visiting patients, verifying plans of care, and delivering nursing care under RN delegation.

Travel up to 50% is required and traveling to patients’ homes is expected. Candidates should have an accredited nursing program credential, an active LVN/LPN license, and at least 1 year of clinical care experience; hospice/home health background preferred.

Competenze

Graduate of an accredited Certificate, Diploma, or Associate School of Nursing.
Current State LVN/LPN license.
1 year of experience in a clinical care setting.
Hospice or home health experience preferred.
50% travel required.

Mansioni

Verifies the Plan of Care prior to each visit and provides care according to delegation.
Incorporates patient care goals into nursing visits with proper documentation.
Communicates significant findings to the RN Case Manager, physician, and others.
Reports unsafe conditions to the RN Case Manager and physician.
Implements patient safety plan using available resources.
Assists patient in learning self-care techniques while delivering quality care.
Suggests revisions to the plan of care and notifies the RN Case Manager.
Provides services with substantial and specialized nursing skills under supervision.
Collaborates with the RN Case Manager during business hours for guidance.
Documents medication regimen and updates profile.
Treats patients and caregivers with respect and professionalism.
Aims to prevent avoidable hospitalizations and ER visits.
Maintains positive communication with physicians, patients, families, and staff.
