Job summary

Acadian HomeCare Lafayette, part of LHC Group and the Optum family, is hiring a Licensed Practical Nurse to deliver high-quality home health care in patients' homes. You will perform clinical tasks within the plan of care, document visits in the EMR and EVV, and collaborate with the care team to coordinate patient needs.

Required LA LPN licensure and a valid driver's license; one year of home health experience is preferred.

Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current driver's license and vehicle insurance, access to dependable vehicle or public transportation.

Responsibilities

• Provides high quality clinical services within the scope of practice per plan of care and infection control procedures.
• Documents patient visits thoroughly in the EMR and EVV system.
• Participates in care coordination, reporting patient needs and changes in condition, and communicating with patient, family, physician and team.
• Able to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition.
