Job summary

Adoration Home Health, part of Adoration Health, is seeking a licensed practical nurse to join our home health team in Elizabethtown, KY. The position is part-time with 1:1 patient care across Hardin, Grayson, Breckinridge and Meade Counties.

You will administer medications, support plans of care, document activities, and collaborate with families and physicians to maintain patient dignity and independence.

Qualifications

• LPN/LPT/LVN license required in state of practice
• Valid driver's license and proof of insurance
• CPR certification
• 1+ years nursing experience preferred
• Home health care experience preferred

Responsibilities

• Provide 1:1 care following the plan of care while preserving patient dignity and independence
• Administer medications, feedings, oxygen, ostomy care as required and assess results
• Document nursing actions and progress, and communicate with family and physicians
• Coordinate supplies, medications, and feedings to ensure availability
• Ensure patient needs align with physician instructions

Responsibilities

As a Home Health Licensed Practical Nurse, You will:

• Providing 1:1 care for client following client's plan of care at all times while maintaining client's dignity and independence
• Administering medications, feedings, oxygen, ostomy care as needed per client's plan of care and provided assessments of results
• Accurately documenting nursing actions of all care given and communication with family, case manager, and physicians as well as documenting the progress and outcomes for established goals and informing physician, case manager, and families of changes in client's medical condition and needs
• Coordinating or ordering supplies, medications, and feedings as needed to insure adequate amounts are always on hand
• Ensuring all patient needs are being met in accordance with the instructions of the physician

Qualifications

• LPN/LPT/LVN Degree from an accredited college of nursing with current unrestricted registration and license in the state of practice and in the state of residence
• Valid driver's license, acceptable driving record and proof of car insurance
• CPR professional certification
• 1+ years of nursing experience preferred
• Home Health care experience preferred
