Job summary

Beacon Home Health in Memphis, TN is seeking an LPN to join our Home Health team. This role focuses on delivering skilled nursing care in patients' homes and coordinating with supervising clinicians to ensure quality outcomes.

The LPN will follow the plan of care, educate families, and monitor patient progress daily. Reliable transportation and compliance with state regulations are essential for success in this field.

Qualifications

  • One year of clinical LPN experience.
  • Current LPN license in the state where services are rendered.
  • Current CPR certification, driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
  • Must be able to reliably commute to multiple patient residences daily within the geographic area assigned.

Responsibilities

  • Provide quality patient care and skilled nursing in the patient’s home as defined by the plan of care.
  • Instruct the patient’s family in home care and maintain a healthy environment for the patient.
  • Observe, record, and report patient response to treatment and changes in condition daily.
  • Collaborate with physicians and RNs, document care accurately and promptly, and do not alter the plan of care.

POSITION OVERVIEW:

The LPN is responsible for providing quality patient care and performing skilled care in the patient’s home. A few position highlights include: Provide services within the scope of practice as defined by the state laws governing the practice of nursing, in accordance with the plan of care, and in coordination with other members of the health care team. Instruct the patient’s family in the care of the patient and maintains a healthy environment for the patient. Observe, record, and report to the supervising nurse and/or physician the patient's response to treatment, and changes in the patient’s condition daily. Collaborate closely with physicians and RNs, documents care accurately and promptly and may not alter the plan of care.

THE MUST HAVES:
