Job summary

Humana Inc. CenterWell Home Health is seeking an LPN to provide one-on-one in-home nursing care, under an RN's supervision, helping patients regain independence.

You’ll implement care plans, educate families, and coordinate with the care team to ensure smooth transitions and positive outcomes for patients under your care.

Qualifications

• Current nursing license in the practicing state.
• Valid driver's license, auto insurance and reliable transportation.
• Current CPR certification.
• Two years experience as an LPN/LVN in a clinical setting.

Responsibilities

• Provide skilled nursing care to patients in their homes under RN direction.
• Implement individualized care plans with patient, family and healthcare team.
• Educate patients and families on disease management and self-care.
• Maintain accurate records of patient visits, interventions, and outcomes.
• Coordinate with physicians and clinical team and participate in discharge planning.
• Maintain prompt communication regarding patient needs and status changes.

Become a part of our caring community As a CenterWell Home Health nurse, reporting to the Clinical Manager, you will provide personalized, one-on-one care that helps patients regain independence in the comfort of their homes. Working with a dedicated team of physicians and clinicians, you'll manage care plans that support recovery and help patients get back to the life they love.

As a Home Health LPN, you will:

• Provide skilled nursing care to patients, working onsite in their homes, under the direction of an RN.
• Implement individualized care plans in collaboration with the patient, family, and healthcare team.
• Educate patients and their families on disease management, treatment options, and self-care techniques.
• Maintain accurate records of patient care, including documentation of patient visit, interventions, and outcomes.
• Maintain contact with patients, physicians, clinical manager(s), and other members of the healthcare team promptly regarding patient or family needs and status changes.
• Participate in care coordination activities and discharge planning.
• Use your skills to make an impact

Required Qualifications

• Current nursing license in the practicing state
• Valid drivers license, auto insurance and reliable transportation
• Current CPR certification
• Two years experience as an LPN/LVN in a clinical setting

Preferred Qualifications

• Nursing experience in a Home Health or Hospice setting

Additional Information TB Statement: This role is considered patient facing and is part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB. Driving Statement: This role is part of Humana's driver safety program and therefore requires an individual to have a valid state driver's license and are expected to maintain personal vehicle liability insurance. Individual must carry vehicle insurance in accordance with their residing state minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event /$10,000 for property damage or whichever is higher.

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc. $53,000 - $73,000 per year
