Job summary

CenterWell Home Health in the United States is seeking an LPN to provide skilled nursing care in patients’ homes under the supervision of an RN. You will implement individualized plans, educate families, and document care and outcomes to support recovery and independence.

We require an active LPN license in the practicing state, CPR, a valid driver’s license with reliable transportation, and at least two years of LPN/LVN clinical experience. Home health experience is preferred.

As a Home Health LPN, You Will

• Provide skilled nursing care to patients, working onsite in their homes, under the direction of an RN.

• Maintain accurate records of patient care, including documentation of patient visit, interventions, and outcomes.
• Maintain contact with patients, physicians, clinical manager(s), and other members of the healthcare team promptly regarding patient or family needs and status changes. Participate in care coordination activities and discharge planning.

Required Qualifications

• Current nursing license in the practicing state

Pay Per Visit/Unit Rate

$33.00 - $47.00

TB Statement

This role is considered patient facing and is part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.

Driving Statement

This role is part of Humana's driver safety program and therefore requires an individual to have a valid state driver's license and are expected to maintain personal vehicle liability insurance. Individual must carry vehicle insurance in accordance with their residing state minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event /$10,000 for property damage or whichever is higher.

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
