Job summary

Aveanna Healthcare in Hernando County, Florida is seeking a Licensed Practical Nurse for part-time, Monday–Friday visits. You will work under a Registered Nurse to deliver skilled nursing care in patients’ homes, following care plans and physician orders.

This is a part-time, in-home role with flexible scheduling in Hernando County. Ideal candidates hold an active LPN license in Florida, CPR and a valid driver’s license, with preferred home health experience.

Qualifications

• LPN license in the state of practice is required.
• CPR certification is required.
• Valid driver’s license is preferred.

Responsibilities

• Deliver quality patient care under the direction of a Registered Nurse.
• Document and communicate with the physician and care team.
• Recognize changes in patient needs and adjust care to prevent or reduce risk.
• Educate clients and families based on individual needs.
• Provide in-home care including assessments, wound care, infusions, catheter care, and PICC line dressing changes.

Essential Job Functions

• Deliver quality patient care in compliance with physician orders under direction of their Registered Nurse teammate.
• Perfom the continuum of hands on client care through documentation and timely communication with the client's physician and other caregivers
• Recognizes changes in patient needs and responses requiring intervention and implements care to prevent risk or reduce risk.
• Educate clients and their family members based on client’s specific needs.
• Provide care in patients home using a variety of skills such as phone triage, patient education, observation/assessment, wound care, infusions, catheter care, PICC line dressing changes etc.

Requirements

• An active LPN License in the state of application
• Valid CPR and Drivers license in the state of application

Preferred

• 1-year LPN experience in a home health or health care setting
• Home Care Home Base exp a plus
