Job summary

Interim HealthCare of Atlanta, GA is seeking a full-time Home Health Licensed Practical Nurse (LPN) to join our Gwinnett County team. You’ll provide compassionate, patient-centered care in home settings while maintaining a healthy work-life balance and competitive pay with a strong benefits package.

As a Home Health LPN, you’ll work with an interdisciplinary team, perform assessments, educate patients and families, and ensure the plan of care is followed.

Qualifications

• GA LPN license is required.
• Minimum of 1 year experience in home health.
• CPR certification is required.
• Valid driver's license and reliable transportation.
• Knowledge of state and federal home health regulations.

Responsibilities

• Provide home-based care under supervision of an RN.
• Collaborate with physicians, RNs and CNAs to deliver care.
• Examine patients, complete assessments and report changes.
• Educate patients on plan of care, medications, therapy, diet and exercise.
• Coach family members on plan of care and home safety.
• Coordinate care with interdisciplinary team and plan for discharge.

As a Home Health Licensed Practical Nurse (LPN), here's a big-picture view of what you'll do:

• Provide home-based care under the supervision of an RN to patients with illnesses, injuries and chronic diseases - often after discharge from a hospital, rehab or nursing facility
• Work with a team of physicians, RN's, and CNAs to deliver quality patient care
• Examine patients, complete assessments, document progress and report changes to their physician
• Educate patients on their plan of care, prescribed medication, therapy, diet and exercise
• Coach family members on the patient's plan of care, medication and home safety
• Collaborate with interdisciplinary team to coordinate patient care and ensure goals are met and plan for discharge from services

A few must-haves for Home Health LPN's:

• Graduate of a practical (vocational) nursing program for LPNand has a current GA LPN license
• Minimum of 1year(LPN) experience in home healthcare
• HCHB charting experience highly preferred
• CPR certification, negative ppd screening or chest x-ray
• Drivers license, reliable transportation & auto insurance
• Knowledge of state and federal home health regulations
• Clinical proficiency in nursing specialty, strong communication skills and compassion
• Able to pass federal and state required criminal / abuse background checks and drug screen
