Adoration Health in Charlotte, NC is seeking a licensed social worker to join our Home Health team. As a Home Health Social Worker, you will provide patient-centered psychosocial assessments, counseling, and care coordination in the home setting.

Qualifications

• Master’s Degree in Social Work required.
• Valid and current Licensed Clinical Social Worker (LCSW) or Licensed Medical Social Worker (LMSW) license.
• Ability to perform psycho-social assessments and develop viable care plans.

Responsibilities

• Guide and counsel individuals and families in crisis.
• Assist clients in obtaining government funds, education, or treatment.
• Secure and evaluate medical, psychological, and social information contributing to client outcomes; assess capacities and problems.
• Help patients and families through conferences regarding plans for meeting needs and mobilizing resources.
• Develop a service plan to restore patient to optimum social and health functioning.
