Choice Health At Home is seeking a Home Health Field Registered Nurse (RN) in Sherman, TX. This full-time role emphasizes patient-centered care delivered in patients’ homes, with strong collaboration across the care team.

You will assess, plan, implement, and evaluate nursing care, educate families, and ensure quality and continuity of home health services. Two+ years of Home Health RN experience and HomeCare HomeBase familiarity are preferred.

Qualifications

• Current RN license is required.
• Valid Driver's License.
• CPR certification.
• Valid Auto Insurance.
• At least 2 years of Home Health RN experience preferred.
• HomeCare HomeBase experience preferred.

Responsibilities

• Initial and ongoing comprehensive assessments and treatment based on patients' needs.
• Determine scope and frequency of services based on acuity and family needs.
• Develop and document the plan of care for quality and continuity of care.
• Educate patients and families on self-care, nutrition, and dietary needs.

About the Role:

The Home Health Field Registered Nurse (RN) plays a critical role in delivering high-quality, patient-centered care within the comfort of patients' homes. This position involves comprehensive assessment, planning, implementation, and evaluation of nursing care tailored to individual patient needs, often managing complex medical conditions. The RN collaborates closely with interdisciplinary teams, patients, and their families to promote health, prevent complications, and support recovery or chronic disease management. By providing education, emotional support, and clinical expertise, the RN empowers patients to achieve optimal health outcomes and maintain independence. Ultimately, this role ensures continuity of care outside traditional clinical settings, improving patient satisfaction and quality of life.

As a Home Health RN , you will:

Initial and ongoing comprehensive assessments and treatment on the patients physical, functional, psychosocial, and environmental needs as evidenced by documentation, clinical record, case conference, team report, evaluations, and ADLs.

Apply specific criteria for admission and re-certification to home health care to establish appropriate levels of care and the patient's eligibility.

Implement, develop, and document the plan of care to ensure quality and continuity of care and recommend revisions to the plan as necessary.

Consult with and educate the patient and family regarding self-care techniques, nutrition, and dietary needs.

Determine scope and frequency of services needed based on acuity and patient/family needs. Assess the ability of the caregiver to meet the patient's immediate needs upon admission and throughout care.

Initiate appropriate preventative and rehabilitative nursing procedures.

Applies concepts of infection control and standard precautions in coordinating and performing patient care activities to prevent contamination and transmission of disease.

Provide clinical directions to the Home Health Aide and LPN/LVN to ensure quality and continuity of service provided.

Prepare clinical and progress notes that demonstrate progress toward established goals.

Inform physician and other personnel of changes in the patient's needs and outcomes of intervention, while evaluating patient and family response to care.

Monitor assigned cases to ensure compliance with requirements of third party payor.

Provide effective communication to patients, family, team members, and other health care professionals.

Requirements

Current RN License
