This role requires in-field visits to patients’ homes throughout Northern California, collaborating with Case Managers and other clinicians to deliver high-quality, compassionate care while advancing toward case management opportunities.

It serves as a foundational role for registered nurses aspiring to advance into case management.

This position works under an Case Manager Registered Nurse position.
