Job summary

WelbeHealth is seeking a Home Care Assistant to provide in-home personal care and support services for participants in their homes across assigned schedules. You will perform ADLs, assist with instrumental tasks, and document vital signs while working as part of an IDT and transportation to medical appointments.

This role offers a stable full-time schedule with base pay plus bonus, travel time and mileage reimbursement, and comprehensive benefits including medical insurance and a 401K match.

Qualifications

• High school diploma or equivalent required.
• BLS/First Aid certification required.
• CNA or HHA certification preferred.
• Bilingual ability preferred; translation support may be needed.

Responsibilities

• Provide hands-on personal care and assistance with activities of daily living (ADLs) in participants' homes.
• Assist with Instrumental ADLs such as laundry, light housekeeping, shopping, and meal prep.
• Measure and document participants’ vital signs.
• Act as a medical escort on vehicles to medical appointments; assist transfers.
• Support Rehabilitation team with home exercise programs per care plan.
• Provide translation services as needed.

## About this roleWelbeHealth PACE helps seniors stay in their homes and communities by providing comprehensive medical care and community-based services. We serve the most vulnerable seniors with better quality and compassion in a value-based model.Our Home Care Assistants are a vital part of our team, providing aid to our participants in their homes as well as contributing to the care planning process as a member of our interdisciplinary team (IDT). Reporting to the Home Care Supervisor, the Home Care Assistant will provide safe delivery of personal care services to WelbeHealth participants in their home, as scheduled and directed.\\*\\*This role is different because Home Care Assistants at WelbeHealth: \\*\\** Work a stable, full-time schedule – unlike traditional home care roles, you will be assigned participants to fill a full workday, and will be re-assigned in cases of cancellations or unforeseen circumstances* Visit 2-5 patients per day depending on participant needs and care plans, working shorter individual shifts that still add up to an 8-hour workday* Receive reimbursement for travel time and mileage between participants**We care about our team members.
