Job summary

Assisteo is seeking a Certified or Licensed Nursing Assistant in Phoenix, Arizona. The role involves providing patient care and observing patients under the supervision of a Registered Nurse.

Job description

The Certified or Licensed Nursing Assistant works under the supervision of a Registered Nurse or Therapist and performs various personal care services as necessary to meet the patient’s needs. The home health aide is responsible for observing patients, reporting these observations and documenting observations and care performed.

Essential Job Functions/Responsibilities

1. Provides direct patient care as defined in the Arizona Nurse Practice Act.
2. Implements plan of care initiated by the Registered Nurse, Physical Therapist or Occupational Therapist.
3. Ability to provide competent hands‑on personal care including:
   • Sponge, tub and shower bath
   • Bed bath
   • Hair shampooing
   • Nail and skin care
   • Oral hygiene
   • Toileting assistance
   • Dressing and undressing
   • Feeding
4. Taking and recording oral, temporal, and axillary temperatures, pulse, respiration and blood pressure when ordered (with appropriate completed/demonstrated skills competency).
5. Assisting in ambulation and exercise according to the plan of care.
6. Performing range of motion and other simple procedures as an extensional therapy service as ordered (with appropriate completed/demonstrated skills competency).
7. Performing simple procedures as an extension of nursing services as ordered (with appropriate completed/demonstrated skills competency).
8. Reporting on patient’s condition and changes to the assigned Registered Nurse.
9. Assisting patient in the self‑administration of medication.
10. Meeting safety needs of patients and using equipment safely and properly (foot stools, side rails, hoyer lift etc.).
11. Homemaker duties may be requested and can include: light house cleaning, changing bed linens, laundry, planning and preparing meals.
12. Ensures protection of confidential information that is stored, handled, or maintained by the Agency per HIPAA regulations.
13. Submits required documentation in an accurate and timely manner within Agency guidelines from time of visit.
14. Understands and practices all State and Federal Laws within the scope of practice.
15. Performs other duties as assigned by the Registered Nurse or Therapist, within the Certified or Licensed Nursing Assistant’s scope of practice.

Additional Duties

1. Ensures that arrangements for equipment and other necessary items and services are available for patient.
2. Uses medical equipment, PPE and supplies effectively and efficiently.
3. Must participate in agency’s sponsored in‑service training.
4. Participates in personal and professional growth and development.

Position Qualifications

1. Certified Nursing Assistant with current state certification or Licensed Nursing Assistant with current state license to practice in AZ.
2. Must have a valid driver’s license and be able to work in the United States.
3. Must have working knowledge of Miscrosoft Office Suite and ability to learn to use Electronic Medical Records Systems.
4. Demonstrates excellent observation, problem solving, organizational, verbal and written communication skills along with the ability to handle multiple tasks using sound judgment and available resources with minimal supervision.
5. Meets and/or exceeds skill requirements per competency checklist.
6. Maintain current CPR certification.
7. Maintain current First Aid certification.
8. Maintain current TB test results.
9. Maintain current fingerprint clearance card with the State.
10. Maintain current auto insurance in accordance with State and/or Agency requirements.
11. Maintain good standing with the Department of Motor Vehicles.
12. Automobile must stay in good working order and is a reliable mode of transportation.
13. Must be able to pass a background check for pre‑employment and ongoing checks per Agency guidelines.
14. Must be COVID‑19 Vaccinated or have a qualifying exemption and agree to Agency tracking process.

Working Environment/Physical Requirements

Prolonged or considerable walking, standing, driving. Able to lift, position, or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling, or crouching. Visual acuity, hearing, dexterity, and attention to detail to perform required nursing skills. Must have ability to perform CPR.
