Job summary

Presbyterian Healthcare Services seeks a Home Health Aide to perform patient care in home settings under the direction of licensed clinicians. You will plan routes, manage caseload and document patient data in the EMR while supporting daily living activities.

Qualifications

• 1 year of direct patient care in any clinical setting, or in-home health/hospice is preferred.
• CNA certification and BLS are required.
• Proficiency with Microsoft Office and EPIC charting system.

Responsibilities

• Provides care in patient home setting under RN or licensed clinician.
• Plans routes to patient addresses and manages caseload to ensure timely care.
• Documents in patient record and records vital signs and patient data as ordered.
• Assists with activities of daily living and medication prompts.
• Follows care plan and participates in IDT conferences.
• Performs simple meal preparation, housekeeping tasks and respite care as ordered.
