Job summary

CenterWell Home Health, part of Humana, seeks a Registered Nurse Clinical Manager to lead clinicians in home-based care and guide clinical practice. You will coordinate patient services, ensure quality outcomes, and empower teams to deliver compassionate care.

Responsibilities include overseeing operations onsite, reviewing referrals and plans of care, educating staff, ensuring documentation and billing compliance, and contributing to hiring, training and budgeting efforts.

Qualifications

• Current state license as a Registered Nurse.
• Proof of CPR certification.
• Valid driver's license with reliable transportation.
• Two years as a Registered Nurse with at least one year of management in a home care, hospice or equivalent environment.

Responsibilities

• Oversee clinical operations for the location, onsite in-office.
• Review referrals, determine admission appropriateness, assign clinicians, and ensure Plans of Care meet standards.
• Guide, support, and educate clinicians on care planning and decision-making.
• Ensure documentation, audits, and billing meet Medicare and payer standards.
• Participate in hiring, training, and performance evaluations for clinical staff.
• Conduct ongoing staff education and quality improvement based on data.
• Coordinate communication among physicians, team members, and caregivers for care coordination.
• Participate in quality improvement, budgeting, marketing and community relations.
• Provide direct patient care in exceptional circumstances and act as Branch Director in absence.
• Perform additional tasks to support clinical operations and organizational goals.
