Role Description

The Home Health Clinical Manager is a full-time, on-site role based in Lake Mary, FL, responsible for overseeing clinical operations for home health services. This role includes managing day-to-day clinical workflows, supervising nursing and clinical staff, and ensuring adherence to clinical protocols and regulatory requirements. The Clinical Manager coordinates patient care plans, reviews documentation for accuracy and completeness, and monitors quality metrics to improve outcomes and patient satisfaction. Daily tasks also involve supporting staff education and training, collaborating with physicians and interdisciplinary teams, and responding to clinical issues or escalations in a timely manner. The role requires consistent on-site presence to support staff, engage with patients and families as needed, and maintain smooth and compliant home health operations.

Qualifications

• Demonstrated experience in clinical operations and home care settings, with the ability to oversee day-to-day workflow and patient care coordination.
• Strong background in medicine or nursing practice, with the skills to guide clinical decision-making and support evidence-based care.
• Proven experience in training and staff development, including coaching clinical teams and implementing educational programs.
• Knowledge of quality improvement principles, including monitoring performance metrics and driving process improvements in home health services.
• Active clinical license (e.g., RN) in the state of Florida preferred, with prior experience in home health or community-based care.
• Excellent communication, leadership, and organizational skills, with the ability to work collaboratively with multidisciplinary teams.
• Familiarity with electronic health records, home health regulations, and payer requirements.
• Bachelor’s degree in nursing, healthcare administration, or a related field preferred; advanced education or certifications in home health management are a plus.
