Job summary

Jethro Health System in Lynn, MA is seeking a Registered Nurse to serve as a Clinical and Admissions Coordinator for our home health agency. The RN will help manage daily operations, coordinate patient intake/admissions, and review clinical information before admission to ensure the best care pathway.

Ideal candidates have an MA RN license, at least 2 years in home health or related settings, and familiarity with Medicare/Medicaid processes.

Qualifications

• RN licensed to practice in Massachusetts.
• Graduate of an accredited RN program.

Responsibilities

• Coordinate referrals from hospitals, physicians and patients with the nursing team.
• Review clinical information prior to patient admission to ensure proper pathway.
• Manage daily operations and act as clinical/admissions coordinator.
• Communicate with nurse manager and teams about new patients.
• Assist with intake, liaison and discharge planning as needed.
• Coordinate weekly staff meetings and ensure regulatory compliance.
• Maintain records of referrals and admissions and verify insurance.
