As a nurse with PurposeCare, you will perform skilled visits, assess health status, and collaborate with caregivers to maintain patient independence and safety in the home setting.

Qualifications

• Current Indiana nursing license is required.
• At least 1 year of nursing experience, home care preferred.
• Must have valid driver's license and reliable transportation.

Responsibilities

• Provide skilled nursing visits to patients at home.
• Assess and monitor patient health status and progress.
• Coordinate care plans with interdisciplinary team.
