The role requires collaboration with caregivers, families, and healthcare professionals to coordinate care and maintain compliance with applicable clinical and documentation requirements.

Qualifications

• Active RN license in MA is required.
• Home care or case management experience is preferred.
• Strong knowledge of clinical documentation and compliance standards.
• Experience with Medicare guidelines preferred.
• Excellent communication and interpersonal skills.
• Strong attention to detail and organizational skills.
• Valid driver’s license and reliable transportation.
• CPR certification required.

Responsibilities

• Conduct patient assessments and develop individualized care plans.
• Review and maintain accurate clinical documentation in compliance with regulatory standards.
• Coordinate care with caregivers, physicians, case managers, and families.
• Monitor patient condition and document changes.
• Ensure care delivery aligns with care plans and clinical needs.
• Support Medicare and state compliance requirements through proper documentation and oversight.
