Qualifications

• Current unrestricted RN licensure in state of practice.
• Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.

Responsibilities

• Provides clinical services within the scope of practice, in accordance with the plan of care.
• Receives referrals, assigns clinicians, evaluates orders, and plots start of care visits.
• Coordinates determination of patient home health benefits, medical necessity, and ongoing insurance approvals.
• Ensures patient needs are assessed and care is individualized per physician orders.
• Reviews assessments and plans of care daily and consults clinicians as appropriate.
• Functions in diverse home situations irrespective of client demographics.
