Job summary

The Lakes Home Care, Inc. seeks a compassionate Home Health Aide to assist patients with daily living activities in their homes according to a care plan.

You will perform personal care, bathing, dressing, feeding, and light household tasks while maintaining patient dignity and safety. Responsibilities include monitoring vital signs as ordered, documenting observations, communicating with the patient, family, and nurse supervisor, and following all safety procedures and state requirements for

Qualifications

• Meets all training requirements in accordance with state and federal laws when applicable
• Ability to read, follow written instructions and document care given
• Possess and maintains current CPR certification in accordance with state regulation
• PPD or chest x-ray if applicable
• Successful completion of Maxim screening examinations
• Participation and completion of Maxim’s competency program

Responsibilities

• Takes and records temperatures, pulse, respiration, and blood pressure when ordered
• Safe transfer and ambulation techniques
• Exercises according to the plan of care
• Performing ROM and other simple procedures per order
• Performs patient laundry and maintains safety practices
• Observes and documents patient condition and reports changes to nurse
• Follows plan of care and completes all tasks
• Communicates clearly with client, family/caregiver and office
• Observes and documents patient condition and reports changes to the supervisor

Job description

The Lakes Home Care is looking for a compassionate Home Health Aide to provide assistance in daily living and personal care services in patients homes in accordance with an established care plan.

Responsibilities:

• Responsible for assisting patients with and per plan of care, the following:

  • Baths, Oral hygiene, Dressing and undressing, Toileting activities, feeding, etc.

  • Taking and recording oral, rectal and axillary temperatures, pulse, respiration and blood pressure when ordered (with appropriate completed/demonstrated skills competency)

  • Safe transfer and ambulation techniques

  • Exercise according to the plan of care

  • Performing range of motion and other simple procedures as an extensional therapy service as ordered (with appropriate completed/demonstrated skills competency)

  • Patient’s laundry

  • Meeting safety needs of patients and using equipment safely and properly (foot stools, side rails, etc.)

  • Observation and teaching of home safety practices

  • Recognition of an emergency situation and knowledge of emergency management procedures

• Must follow patients’ plan of care and complete all tasks assigned

• Communicates in a constant, clear, and effective way with the client, family/caregiver and office

• Observes and documents patient condition and reports any changes, to the assigned nurse supervisor
