Job summary

The Lakes Home Care, Inc. is seeking a compassionate Home Health Aide to assist patients in their homes per an established care plan. The role focuses on daily living and personal care tasks with a patient-first approach.

You will follow written care plans, report changes to the nurse supervisor, and help with safety, transfers, and basic therapies while maintaining patient dignity and comfort in a part-time schedule.

Qualifications

• Meets all training requirements per state and federal laws
• Ability to read, follow written instructions and document care
• Currently CPR certified and maintain certification as required

Responsibilities

• Assist patients with activities of daily living per plan of care (baths, hygiene, dressing, toileting, feeding)
• Take and record vitals as ordered (temp, pulse, respiration, BP)
• Support safe transfers and ambulation
• Assist with prescribed range of motion and simple exercises
• Handle patient laundry and maintain safety practices at home
• Observe and report patient conditions to the supervising nurse

Job description

The Lakes Home Care is looking for a compassionate Home Health Aide to provide assistance in daily living and personal care services in patients homes in accordance with an established care plan.

Responsibilities:

• Responsible for assisting patients with and per plan of care, the following:

  • Baths, Oral hygiene, Dressing and undressing, Toileting activities, feeding, etc.

  • Taking and recording oral, rectal and axillary temperatures, pulse, respiration and blood pressure when ordered (with appropriate completed/demonstrated skills competency)

  • Safe transfer and ambulation techniques

  • Exercise according to the plan of care

  • Performing range of motion and other simple procedures as an extensional therapy service as ordered (with appropriate completed/demonstrated skills competency)

  • Patient’s laundry

  • Meeting safety needs of patients and using equipment safely and properly (foot stools, side rails, etc.)

  • Observation and teaching of home safety practices

  • Recognition of an emergency situation and knowledge of emergency management procedures

• Must follow patients’ plan of care and complete all tasks assigned

• Communicates in a constant, clear, and effective way with the client, family/caregiver and office

• Observes and documents patient condition and reports any changes, to the assigned nurse supervisor

Requirements:

• Meets all training requirements in accordance with state and federal laws when applicable

• Ability to read, follow written instructions and document care given

• Possess and maintains current CPR certification in accordance with state regulation

• PPD or chest x-ray if applicable

• Successful completion of Maxim screening examinations

• Participation and completion of Maxim’s competency program
