Qualifications

Responsibilities

• Maintain open communication with case manager and Team Leader regarding health, safety and environment status of patients.
• Comply with State/Federal regulatory acts.
• Perform tasks as prescribed by the home health aide plan of care.
• Submit time sheets and documentation timely and accurately.
• Provide emotional support to patients and families.
• Report concerns of potential abuse or neglect to supervisor.
• Encourage patient independence through participation in personal care and activities.
