Job summary

CenterWell Home Health is seeking a Home Health Aide to support patients at home under the direction of the Associate Director, Branch Administration. You will help patients regain strength, mobility and independence through rehabilitation-focused care, enabling daily activities with confidence and improved quality of life.

Responsibilities include direct patient care, medication assistance, vital signs, hygiene support, feeding, mobility and documentation.

Qualifications

  • Complete Certified Nursing Assistant or Certified Home Health Aide Program within the last 24 months.
  • Meet applicable state certification requirements.
  • Must have a valid driver's license, auto insurance, and reliable transportation.
  • Must be in good standing on the HHA Registry (if applicable) and completed HHA/CNA course for Medicare agency.
  • Minimum one year experience as a Home Health Aide or CNA in the last 24 months.

Responsibilities

  • Provide direct patient care under RN supervision and according to the POC.
  • Assist with self-administered medications and monitor dosing.
  • Take vital signs and report abnormal results.
  • Assist with personal hygiene and activities of daily living.
  • Support feeding and observe nutrition; assist with mobility and exercises.
  • Assist with toileting and ostomy care as needed.
  • Document care provided and communicate with the care team.
  • Maintain infection control and create a safe environment.
  • Assist with light household tasks when needed and allowed.

Job Overview

As a Home Health Aide reporting to the Associate Director, Branch Administration at CenterWell Home Health, you will help patients regain strength, mobility and independence — all from the comfort of their homes. By delivering personalized care focused on rehabilitation and functional improvement, you’ll empower individuals to overcome physical limitations, perform everyday activities with confidence and enjoy a better quality of life.

Responsibilities
  • Provide direct patient care to patient under the direction of the RN and according to the Aide Plan of Care (POC).
  • Assist the patient with self-administered medications by opening bottle caps for the patient, reading medication labels to the patient, checking the dose being self-administered against the prescribed dose on the container label and observing the patient take the medication.
  • Take accurate temperature, pulse and blood pressure measurements and recognize and report abnormal results to supervisor.
  • Help patient maintain good personal hygiene by performing or supervising bathing, grooming, skin care, shaving, oral care, nail/foot care and other activities.
  • Assist in feeding patients and communicate basic principles of nutrition, observe and record food and fluid intake. Safely position patient for meals and feed or support self-feeding.
  • Assist with patient toileting including use of bed pan/urinal, change and position catheter bags and bag change procedures on well-regulated ostomies.
  • Provide necessary skills to safely assist the patient with patient mobility, exercises, positioning/turning, transfers and ambulation per Plan of Care and CenterWell Home Health policy.
  • Report changes and document pertinent information and care rendered to patient to ensure continuity of care. Document interactions with patients, caregivers, doctors and other staff members appropriately, legibly, thoroughly and in the amount of time allowed.
  • Practice acceptable infection control principles. Provide a clean, safe and comfortable environment.
  • Help with other household tasks, including light laundry, bed changing and bed making, light meal preparation, light housekeeping and shopping (if no other assistance is available and an MD order is present).
Required Qualifications
  • Complete Certified Nursing Assistant or Certified Home Health Aide Program within the last 24 months.
  • Must meet applicable state certification requirements.
  • A valid driver's license, auto insurance, and reliable transportation are required.
  • Must be in good standing on the HHA Registry (if applicable) and have completed HHA/CNA course to work for a Medicare certified agency.
  • Minimum one year experience in the last 24 months as a Home Health Aide or Certified Nursing Assistant in a hospital, nursing home, home health/hospice agency.
Additional Requirements
  • TB Screening: This role is patient-facing and part of Humana's Tuberculosis screening program. If selected, you will be required to be screened for TB.
  • Driver Safety: This role is part of Humana's driver safety program. You must have a valid state driver's license and maintain personal vehicle liability insurance with state minimum required limits (minimum coverage typically $25,000 bodily injury per person/$25,000 bodily injury per event /$10,000 property damage or higher).
