Job summary

BayCare is seeking a Home Health Aide per Visit to deliver compassionate in-home care in Pinellas County. The role is hourly, per-visit, with a schedule that includes shifts from 7:00 AM to 7:00 PM and every 3rd weekend. On-call is not required.

Responsibilities include assisting with personal care, following the plan of care, maintaining clear communication with the patient and family, and ensuring infection control standards are met.

Qualifications

• CNA or HHA certification from a 75-hour program.
• Florida driver's license required within 30 days of hire.
• Basic Life Support (BLS) certification.

Responsibilities

• Provide personal care to patients under supervision of a nurse.
• Follow the patient's plan of care and complete assigned tasks.
• Maintain open communication with patient, caregivers, and family.
• Demonstrate competency with assigned tasks.
• Comply with infection prevention and control policies.
• Report changes in patient condition and honor patient rights.

Position Details:

• Facility: BayCare Health System, Private Duty
• Location: Pinellas County
• Status: Per Visit, hourly
• Shift Hours: 7:00 AM - 7:00 PM, Varies
• Weekend Work: Every 3rd weekend
• On Call: No

Home Health Aide key responsibilities:

• As part of the patient care team, works under the guidance and supervision of the staff nurse in providing personal care to the patients
• Follows the patient's plan of care for completion of tasks assigned to a home health aide by the registered nurse or other appropriate skilled professional
• Maintains an open communication process with the patient, representative (if any), caregivers, and family
• Demonstrates competency with assigned tasks
• Complies with infection prevention and control policies and procedures
• Reports changes in the patient's condition
• Honors patient rights

Required Certifications and Licensures:

• CNA (Certified Nursing Assistant) or HHA certification from accredited 75-hour program
• Driver's License State of Florida (Florida driver's license is required within 30 days of hire)
• BLS (Basic Life Support)

Education:

• Preferred: High School Diploma or equivalent

Experience:

• Preferred 1 year Home Care
