Job summary

Staten Island Care Center in Staten Island, NY is seeking a compassionate Home Health Aide (HHA) to join our team on a PRN basis. You will assist residents with transportation to medical appointments, support safety during outings, and provide a caring presence throughout the day.

Current NYS HHA certification required. Strong communication, dependability, and a patient, resident-centered approach are essential.

Responsibilities

• Escort residents to and from clinic appointments and dialysis sessions.
• Monitor residents to prevent falls or altercations, intervening promptly as needed.
• Assist residents in preparing for appointments and ensure snacks are provided during out-on-pass periods.
• Report all incidents and accidents promptly to the Clinical Coordinator or ADNS.
• Represent the facility in a professional and courteous manner, upholding all policies and procedures both on and off-site.
