Job summary

PACE in Milford, Delaware is seeking a compassionate Home Health Aide (HHA) to join our full-time team. You will provide in-home care focused on physical, emotional, and social well-being, helping patients maintain independence and quality of life.

Responsibilities include personal care, vital signs, daily living activities, medication support, and thorough documentation. A high school diploma is required; Delaware CNA/HHA certification is a plus.

Qualifications

• High School Diploma or equivalent required.
• Preferred CNA or HHA certification in Delaware.
• Ability to lift, transfer, and reposition patients per guidelines.

Responsibilities

• Provide direct patient care including personal care and vital sign monitoring.
• Assist patients with daily living activities such as bathing, dressing, and grooming.
• Support medication management, administration, and tracking.
• Maintain accurate records of patient care, treatment plans, and progress.
• Collaborate with healthcare providers and families to ensure comprehensive care.
• Respect patient rights and dignity, accommodating individual preferences.

Requirements:

• High School Diploma or equivalent
• Nice to have current certification as a Certified Nursing Assistant (CNA) or Home Health Aide (HHA) in the state of Delaware
• Ability to lift, transfer, and reposition patients according to established guidelines
• Excellent communication and interpersonal skills
• Organizational skills with attention to detail and accuracy
• Ability to work effectively in a fast-paced environment, prioritizing tasks and managing time efficiently
