Job summary

Hospice Buffalo is seeking a part-time Home Health Aide (HHA) to support our inpatient and home care patients in Cheektowaga, NY. The role involves assisting with hygiene, daily living activities, meals, and basic nursing tasks under professional supervision.

The ideal candidate will have a NYS-approved HHA training, a high school diploma or GED, and a valid driver’s license. Hospice experience is encouraged, and the schedule includes two shifts per week with weekend and holiday rotations.

Qualifications

• Completion of a NY State approved Home Health Care training program with a Home Health Aide Certificate.
• High school diploma, GED preferred.
• Certification in Hospice and Palliative Care encouraged.
• Must have current Driver's License, free of any serious violations, and vehicle required for position.

Responsibilities

• Assists with hygiene and grooming, including bathing, hair shampoo, nail and skin care, oral hygiene, toileting and related tasks.
• Assists with activities of daily living and instrumental activities of daily living (e.g., answering phone, doing laundry).
• Meets nutritional needs through meal planning, shopping, preparation and feeding when necessary.
• Maintains confidentiality per HIPPA and policy.
• Demonstrates proper hand hygiene, PPE use, and infection control procedures.
• Explains duties and roles of the Interdisciplinary Team to patients and families.
• Documents to agency standards and prioritizes work effectively.
• Communicates with patients and families in a supportive, culturally appropriate manner.
• Ensures a clean, safe environment and safe transfer techniques.

Part-time (2 shifts/week) 7am-3pm with every other weekend rotation and holiday rotation.

Our Inpatient Unit on the Mitchell Campus in Cheektowaga, NY, provides a relaxing home-like setting for select patients and their families. Our unique unit is only offered to patients whose pain or symptoms cannot be managed at home or are approaching their final stage. Hospice patients can also be admitted to the unit for short-term respite care.

Essential Duties and Responsibilities for a Home Health Aide (HHA):

Assists with appropriate hygiene and grooming inclusive of bathing, hair shampoo, nail & skin care; oral hygiene; toileting/assistance with elimination; and weights. Assists with health related tasks and personal care procedures.

Assists with activities of daily living and instrumental activities of daily living (i.e.) answering phone and doing laundry.

Meets the patient's general nutritional needs through meal planning, shopping, meal preparation and clean up and feeding the patient if necessary. Assists in the meal preparation for individuals with special dietary problems under the direction of a professional staff member. Understands adequate nutrition and fluid intake.

Demonstrates effective observation and reporting skills. Maintains high level of confidentiality in compliance with HIPPA law and policy.

Demonstrates appropriate hand hygiene; waste disposal; use of Personal Protective Equipment; Bag (supply/tote) technique; and knowledge of Standard Precautions, inclusive of basic infection control procedures.

Appropriately explains duties and functions of the Interdisciplinary Team including the role of the Home Health Aide.

Documents to agency standards.

Displays the ability to establish priorities and organize work and time appropriately. Anticipates problems and modifies plans and procedures to provide solutions.

Elicited no founded patient/family complaints, staff complaints or unusual occurrences (i.e. reportable incidents).

Demonstrates ability to communicate with patients and families in a supportive manner that is understandable and culturally appropriate.

Maintains a clean, safe environment; provides assistance with safe transfer techniques, positioning and ambulation. Demonstrates normal range of motion.

Assists the patient with taking self-administered medications

Able to read and record temperature, pulse and respiration.

Recognizes emergencies and is knowledgeable in applying emergency procedures.

Is respectful of the patient, his or her privacy and his or her property.

Qualifications for a Home Health Aide:
