Job summary

Health Quest Home Care in Poughkeepsie, NY is seeking a Home Health Aide to provide compassionate personal care, assist with mobility, meals, and daily activities under supervision of a registered case manager.

The role requires NYS Home Health Aide certification and a valid driver’s license, with a sign-on bonus and comprehensive benefits including PTO and health/Dental coverage. Expect active engagement in patient-specific care plans.

Responsibilities

• Assists the patient with personal care activities including bathing, oral hygiene, feeding and dressing.
• Assists with exercises, ambulation, and transfer activities using assistive devices.
• Independently assists with meal preparation and feeding.
• Maintains sanitary kitchen conditions and observes dietary restrictions.
• Provides medication prompts and reminds the patient to self-administer medications.
• Identifies changes in patient condition and notifies the registered case manager; takes vital signs.

Salary Range:$19.50-$25.65

Responsibilities:

• Assists the patient with personal care activities including bathing, oral hygiene, feeding and dressing..
• Assists the client with exercises, ambulation, and transfer activities. This includes knowledge in use of assistive devices such as canes, walkers, crutches, and lifts.
• Independently assists the patient with meal preparation/cooking and/or feeding.
• Must have working knowledge of special diets, diet restrictions, and techniques to maintain sanitary kitchen conditions to prevent food-borne illnesses.
• Assists the patient with medication prompts to remind the patient to self-administer his/her medications.
• Identifies changes in patient condition that require notification to the patient’s registered case manager.
• Takes vital signs to include temperature, pulse, heart rate, respiratory rate.

Required:

• Active NYS, Home Health Aide Certification. Drivers License.
