Team Select Home Care is seeking a Home Health Aide to provide in-home care for medically fragile children under the supervision of an RN. You will perform personal care, assist with activities of daily living, and document care per agency policy.

Role requires CPR, AHCA screening, and completion of 86 hours of training; 18+ years old. Benefits include flexible scheduling, paid training, and weekly pay.

Qualifications

• Related provider, legal guardian or caretaker status for Florida Medicaid recipient.

Responsibilities

• Provides personal and basic care as ordered in the plan of care.
• Performs simple nursing tasks under RN direction.
• Assists with ambulation and exercises as assigned.
• Assists with medications ordinarily self-administered as assigned.
• Completes timely visit records and documents care observations.
• Attends staff meetings and in-service programs as scheduled.
• Maintains confidentiality and professional conduct.

Job description

A Home Health Aide for Medically Fragile Children is a paraprofessional who is specifically trained, competent and performs assigned functions of personal and skilled care to the patient in their residence under the direction, instruction and supervision of the registered nurse (RN). In this role, you will report directly to the Director of Clinical Services/Nursing Supervisor/RN.

Supervisory Responsibilities: No Supervisory Responsibilities

Duties/Responsibilities:

• Provides services that are ordered in the plan of care and that the aide is permitted to perform under state law
• Performs hands-on personal care
• Performs simple procedures as an extension of the nursing services as assigned
• Assistance in ambulation or exercises as assigned
• Assists with medications that are ordinarily self-administered as assigned
• Understands and adheres to established Agency policies and procedures
• Completes appropriate visit records in a timely manner as per Agency policy
• Reports changes in the patient's condition and needs to the appropriate personnel
• Performs household services essential to health care in the home as assigned
• Attends staff meetings and patient care conferences as scheduled
• Follows Agency policy for cleaning equipment between patient uses
• Carries out, reports and documents care given in an effective, timely manner as observed by the RN and through periodic record reviews
• Completes all work assigned by the RN
• Observes confidentiality and safeguards all patient related information
• Reports any variance, accident or unusual occurrence to the appropriate personnel
• Participates in QM activities as requested
• Maintains acceptable attendance status, per Agency policy
• Reports incomplete work assignments to RN
• Appearance is always within Agency standard; is clean and well groomed
• Demonstrates effective time management skills through daily documentation and infrequent overtime for routine assignments
• Attends position related in-services.
• Attends all mandatory in-service programs as scheduled; minimally 12 hours/year
• Maintains clean and neat work environment
• Demonstrates sound judgment and decision making
• Performs other duties as assigned

Required Skills/Abilities/Knowledge:

• Ability to carry out directions, read and write
• Ability to effectively communicate to demonstrate competency in the provision of care and services safely and effectively and treat patients with dignity and respect to person and property
• Maturity and ability to deal effectively with the demands of the job

Education/Experience/Licenses/Certifications/Requirements:

• Maintains current CPR certification
• Maintains clear level 2 AHCA Screening
• Must be at least 18 years of age
• Must complete 86 hours of training
• Must be a related provider, legal guardian or caretaker to an eligible Florida Medicaid recipient

Work Limitation per Program:

• Able work up to 40 hours per week
• Able to work up to 8 hours per day

Pay Range: $12 - 13/ hour
