Job summary

Harmony Home Health and Hospice, LLC. is seeking a compassionate Home Health Aide to join our team in Albuquerque. You will work one-on-one with patients in their homes, following a personalized care plan and building meaningful relationships.

The role requires graduation from an accredited Home Health Aide program, a high school diploma or GED, and CPR certification; driver’s license is needed. Prior home health or hospice experience is a plus.

Qualifications

• High School Diploma or GED.
• CPR certification required.

Responsibilities

• Assist patients with daily activities and mobility.
• Provide personal care and help with daily tasks in the home.
• Maintain accurate records of patient care and report observations to supervisor.
• Build rapport with patients to support emotional well-being.
• Administer medications and immunizations as directed.
• Engage patients in exercises and activities to promote health.
