Graceworks Lutheran Services is a not-for-profit organization serving over 2000 older adults and adults with disabilities. Graceworks at Home/Health at Home provides personal care in clients’ homes to support independence and well-being.

This part-time day shift position offers pay starting at $13 per hour with additional mileage pay. Requires CNAs or equivalent experience and a valid driver’s license. Pre-employment drug screen is required.

Qualifications

• One year of verifiable nurse assistant or home health aide experience in a supervised setting.
• Certified Nursing Assistant or equivalent experience.
• Drug free workplace with pre-employment drug screen requirement.
• Valid driver’s license and safe driving record.

This will be a Part-Time day shift position Pay: $13 and up (additional competitive mileage pay)

Position Summary:

Provides Personal Care Services to clients when necessary to maintain their optimum level of functioning assisting with activities of daily living within Graceworks at Home/Health at Home services. Continually works to promote Client independence

Job duties:

• Provides scheduled personal care needs of the highest quality to clients in their living environment under the Graceworks at Home/ Health at Home Care Program.
• Under the direction of a registered nurse or therapist, provides care in response to the outlined care plan in the home. This includes; hands on care, assisting with medications reminders that are ordinarily self-administered, assistance in ambulation or exercise, and the performance of simple procedures as an extension of therapy or nursing services, as permitted by State of Ohio practice acts.
• Attempt to meet the physical, emotional and spiritual needs of clients.
• Complete paperwork needed to support services and billing requirements.
• Attend scheduled in-services, training hours and staff meetings.
• Responsible for promptly reporting any changes in client’s condition (such as skin breakdown, weigh changes, etc), incidents, theft, damage, and/or abuse to the Administrator of Health at Home, LLC
• Adhere to all policies and procedures of the organization and all federal and state laws.
