Job summary

Always Compassionate Home Care is seeking a dedicated Home Health Aide in NY to assist clients per care plan. You’ll monitor conditions, support daily activities, and help with meals, hygiene, and medications under supervision. Must have NYS PCA/HHA certification, be at least 18, and bring a compassionate, reliable approach.

Qualifications

Responsibilities

• Monitor patient condition by observing physical and mental condition, intake and output and reporting changes to nursing supervisors.
• Maintain a healthful, safe environment.
• Provide nutritional and environmental support.
• Assist with personal hygiene, toileting, dressing, walking and feeding the client.
• Position the client as per care plan.
• Assist patients by providing personal services, such as bathing, dressing and grooming
• Support patient by providing housekeeping and laundry services, shopping and errands as per care plan.
• Prepare and serve meals and snacks as directed by care plan.
• Assist with certain treatments and therapy as ordered by the physician and approved by the nursing supervisor.
• Remind the client to take medications
• Assist patient with shopping efficiently using patient’s money and observing cost-saving techniques.
• Obtain vitals, report abnormal values, document and inform nurse supervisor.
• Use Electronic Visit Verification for daily attendance and timesheet completion.
• Maintain a safe and secure environment by infection control protocol
• Follow dietary requirements as per care plan.
• Use medical supplies and equipment correctly.
• Provide emotional support and build trust with client.
• Encourage client independence.
• Protect agency by following policies and laws.

• Assist patient with shopping efficiently utilizing patient’s money and observing cost-saving techniques.
• Obtaining patients vitals, understanding values not within normal limits, documenting them accordingly and informing the nursing supervisor as required.
• Utilizing Electronic Visit Verification for daily attendance and completion of timesheet
• Maintains a safe and secure environment by following infection control protocol
• Following dietary requirements and nutrition standards as per care plan.
• The correct use of medical supplies and equipment when appropriate.
• Emotional support and the development of a trusting relationship between the aide and the client.
• Encouragement of client independence.
• Protects the agency by following professional standards, home care policies and procedures, federal, state and local requirements.
