Qualifications

• Must have a valid driver's license and reliable transportation.
• Ability to travel to patients' homes as needed.
• Compassionate, patient-centered approach to care.
• Reliable, responsible, and professional work ethic.
• Strong communication and interpersonal skills; teamwork optional.
• Home care experience preferred, but not required.

Responsibilities

• Travel to patients' homes in Fairfield and New Haven Counties.
• Provide personal care and assistance to patients.
• Assist with bathing, grooming, dressing, and personal hygiene.
• Assist with toileting and incontinence care.
• Support safe mobility, ambulation, and transfers.
• Assist patients with prescribed exercises and activities.
• Perform light housekeeping related to patient care and safety.
• Provide companionship and emotional support.
• Observe and report changes in patient condition to supervising nurse.
• Maintain accurate documentation and follow agency policies.

Position Summary
