Job summary

CareDiem Home Health in Rockford, IL seeks compassionate Home Health Aides to help clients stay safely at home with personalized care. You will provide personal care, assist with medications, ambulation, and daily living activities while documenting observations for the RN.

We value reliability and strong communication. Requirements include HHA training, active Illinois State Registry, CPR (BLS), and 1+ year home health experience, plus reliable transportation and a valid driver’s license.

Qualifications

• HHA training and competency evaluation required.
• Must be on Illinois State Registry.
• 1+ year of experience, home health preferred.
• CPR certification (BLS) required.
• Reliable transportation, valid driver's license, and auto insurance.

Responsibilities

What You'll Do:

• Provides Personal Care
• Perform simple procedures as an extension of therapeutic services
• Ambulation and exercise of the patient
• Household services essential to health care at home
• Assistance with medications that are ordinarily self-administered
• Reporting changes in the patient's condition and needs to the registered nurse
• Completion of appropriate clinical records to record care given including observations and treatments.

What We're Looking For:

• Training as a Home Health Aide and completed competency evaluation is required
• HHA must currently be on the Illinois State Registry
• 1+ year experience (home health preferred)
• CPR certification (BLS)
• Reliable transportation, valid driver's license, and auto insurance
• Strong judgement, communication, and independence
