Job summary

Addus HomeCare in Kingston, TN is seeking a Home Health Aide to provide compassionate, hands-on care in patients' homes. You will assist with daily living activities, prepare meals, transport clients to appointments, and support mobility as outlined in the plan of care.

Responsibilities include documentation and reporting patient changes to the supervising nurse, while maintaining confidentiality and respecting HIPAA guidelines. Reliable transportation and a current driver�s license are required.

Qualifications

• One year of hospital or nursing home experience.
• Must be 18+ and a high school graduate or equivalent.
• Ability to read and follow written instructions and document care given.
• Displays empathy for the needs of the ill and frail.
• Demonstrates tact, patience, and good personal hygiene.
• Possess and maintain CPR certification, if required.
• Must have reliable transportation, valid driver�s license and insurance; or ability to take public transportation.

• Providing hands-on personal care including baths, back rubs, oral hygiene, shampoos, changing bed linen, helping with dressing.
• Planning and preparing nutritious meals.
• Assisting in feeding the patient as needed.
• Transport client to doctor�s appointments and essential errands.
• Assisting in ambulation and exercise per care plan.
• Assisting in self-administration of medications.
• Perform other duties as assigned.

What You�ll Do:

• Providing hands-on personal care including baths, back rubs, oral hygiene, shampoos, changing bed linen, assisting patients with dressing and undressing, skin care to prevent breakdown, assisting the patient with toileting activities, and keeping patient�s living area clean and orderly, as appropriate.


• Planning and preparing nutritious meals.


• Assisting in feeding the patient, if necessary.


• Transport client to doctor�s office, grocery store and other essential errands.


• Assisting in ambulation and exercise according to the plan of care.


• Assisting patient in the self-administration of medication.


• Doing patient�s laundry, as appropriate.


• Meeting safety needs of patients and using equipment safely and properly (foot stools, side rails, etc.).


• Reporting on patient�s condition and significant changes to the assigned nurse.


• Maintain a high degree of confidentiality at all times due to access to sensitive information.


• Maintain regular, predictable, consistent attendance and is flexible to meet the needs of the department.


• Follow all Medicare, Medicaid, and HIPAA regulations and requirements.


• Abide by all regulations, policies, procedures and standards.


• Perform other duties as assigned.

Qualifications:

• One-year experience in hospital or nursing home setting.


• At least 18 years of age and is a high school graduate or equivalent.


• Ability to read and follow written instructions and document care given.


• Displays initiative and able to work with minimal direct supervision.


• Empathy for the needs of the ill, frail, and impaired.


• Demonstrates tact, patience, and good personal hygiene.


• Possess and maintain valid CPR certification, if required.


• Must have reliable transportation, current driver's license and appropriate automobile insurance; or ability to take public transportation
