Job summary

PurposeCare is seeking compassionate caregivers to provide one-on-one home health and home care services. You will support daily living activities, help clients stay safe and independent, and travel to patient homes across your service area

Ideal candidates are dependable, patient, service-driven, and willing to obtain certification; flexible scheduling options are available to fit market needs.

Qualifications

• Active HHA or CNA certification required or willingness to obtain
• Ability to follow written care plans and document services accurately
• Strong communication with patients, families and supervising nurse
• Ability to lift up to 25 pounds and perform mobility aid tasks
• TB testing, background screening, and onboarding requirements will be completed

Responsibilities

• Provide personal care such as bathing, grooming, dressing, oral hygiene, and toileting
• Assist with mobility, transfers and range-of-motion exercises per care plan
• Prepare meals and assist with feeding as needed
• Support medication self-administration through reminders (no independent med administration)
• Monitor and report patient condition to the supervising nurse
• Take and record basic vital signs when trained
• Maintain a clean, safe, and orderly client environment
• Travel to patient homes and schedule visits across your service area
• Complete required in-service training, including dementia education

Job description

You'll join a compassionate, fast-growing home health and home care team that supports patients one-on-one in their homes and communities. This is a role for someone dependable, patient, and service-driven who wants meaningful work and flexible opportunities. Key responsibilities

• Provide personal care support including bathing, grooming, dressing, oral hygiene, and toileting
• Assist with mobility, transfers, ambulation, and range-of-motion exercises per care plan
• Prepare meals and assist with feeding as needed
• Support medication self-administration through reminders (no independent med administration)
• Monitor and report patient condition and changes to the supervising nurse
• Take and record basic vital signs when trained and assigned
• Maintain a clean, safe, and orderly client environment
• Assist with linens and light client laundry as appropriate
• Use safety equipment properly (rails, supports, stools, etc.)
• Document care provided according to company and regulatory standards
• Travel reliably to patient homes and assigned visits across your service area
• Complete required in-service training, including annual dementia education

Must-have requirements

• Age 18 or older
• Active HHA or CNA certification or willingness to obtain certification
• Willingness to complete competency and skills assessments
• Ability to follow written care instructions and document services
• Compassion, patience, and professionalism with vulnerable populations
• Ability to work independently in home settings
• Reliable transportation and ability to travel to client homes
• Ability to lift up to 25 pounds
• Willingness to complete TB testing, background screening, and onboarding requirements

Nice-to-have

• 6+ months of caregiving or direct patient care experience
• Home health or home care experience
• CPR and Adult First Aid certification
• Experience supporting seniors or medically complex clients
• Experience working with dementia or memory care patients

Work environment & schedule

• One-on-one care in patient homes and occasional facility settings
• Flexible scheduling options may be available (full-time, part-time, PRN depending on market)
• Assignments may include homes with pets or smokers
• Travel within your preferred coverage area
