Job summary

Harmony At Home is seeking a compassionate Home Health Aide to provide in-home, one-on-one care following a personalized plan. You will work with patients, building relationships and ensuring a healthy, comfortable environment. Strong communication is essential; prior home health or hospice experience is a plus.

Responsibilities include assisting with daily activities, maintaining care records, and engaging patients in activities. A valid driver’s license is required to reach patient homes.

Qualifications

• High School Diploma or GED preferred.
• Must be 18 years or older.
• One year prior professional experience preferred.
• Driver’s license required.
• CPR certification optional.

• Assist patients with daily activities such as moving in/out of beds, baths, wheelchairs, or cars.
• Care for patients by changing linens, doing laundry, cleaning the home, or assisting with personal care.
• Maintain records of patient care, condition, progress, or problems to discuss with the supervisor.
• Build relationships with patients by conversing or reading aloud to keep them mentally healthy and alert.
• Give medications and immunizations.
• Engage patients in exercises or other activities.

We are looking for a Home Health Aide to join our team! You will directly work with patients, following a one-on-one care plan in patient homes. You operate with a big heart, not only providing personalized and attentive care but by building genuine relationships with those you serve. You are detail-oriented, highly organized, and committed to creating a healthy environment that meets patient needs. Strong verbal and written communication skills are needed to succeed in this role. Prior Home Health or Hospice experience is a plus.
