Harmony at Home in Tennessee is seeking a compassionate Home Health Aide to join our team. You will work directly with patients in their homes following a one-on-one care plan, delivering personalized and attentive care while building trusting relationships.

You should be detail-oriented, highly organized, and capable of communicating clearly, both verbally and in writing. Prior Home Health or Hospice experience is a plus, and a valid driver’s license is required to visit patients in their

• High School Diploma or GED preferred.
• Must be 18 Years or Older.
• One year prior professional experience preferred.
• Driver’s license required.
• CPR certification optional.

Responsibilities

• Assist patients with daily activities such as moving in and out of beds, baths, wheelchairs, or automobiles
• Care for patients by changing bed linens, doing laundry, cleaning the home, or assisting with personal care
• Maintain records of patient care, condition, progress, or problems to report and discuss observations with the supervisor
• Build relationships with patients by conversing or reading aloud to help keep them mentally healthy and alert
• Give medications and immunizations
• Engage patients in exercises or other activities
