UnitedHealth Group is seeking a Per diem Home Health Aide to provide hands-on personal care to patients in their residences, supporting comfort, dignity, and daily functioning under supervision of licensed staff. Qualifications

• Current state certification as nurse assistant per state requirements.
• CNA experience within the last two years; must complete training if longer.
• Must pass the competency program of the care center.
• Current CPR certification.

Responsabilités

• Assist patients with activities of daily living (ADLs) such as bathing, dressing, and grooming.
• Obtain and monitor vital signs (temperature, pulse, BP, respiration).
• Maintain a clean, safe, healthy home environment.
• Assist with meals and light meal prep.
• Perform light homemaking and environmental services.
• Observe and report changes in patient status to licensed staff.
• Follow plan of care and instructions from nurses or therapists.
• Support patient safety and adhere to infection control procedures.
• Recognize emergencies and implement appropriate procedures per policy.
• Document changes and activities in patient records.
• Assist with prescribed exercises as instructed by clinician.

As the Per diem Home Health Aide you will provide hands‑on personal care and assistance to patients in their residences, supporting comfort, dignity, and daily functioning. Your role assists with activities of daily living under the supervision of licensed clinical staff while adhering to individualized care plans. As the Home Health Aide you will play an important role in promoting patient safety, independence, and overall quality of life.

Primary Responsibilities:

• Obtains temperature, pulse, blood pressure and respiration
• Maintains a clean, safe, and healthy environment
• Assists patient with getting in and out of bed, bathing, hygiene, and toileting
• Prepares and provides and/or assists with meals
• Performs light homemaking and other environmental services
• Observes, reports, and documents any changes in patient status
• Understands basic elements of body functioning and reports changes in patient’s body functions as indicated
• Recognizes emergency situations and implements appropriate emergency procedures per care center policy
• Follows care center procedures regarding infection prevention and control, handling of hazardous waste and safety measures
• Follows specific written instructions for personal care in accordance with the plan of care
• Assists patient with prescribed exercises as instructed per nurse or therapist
• Performs other duties as assigned

Required Qualifications:

• Current state certification (or registration) as a nurse assistant as per state certification/registration requirements
• CNA experience within the last two years. If it has been more than two years, you must complete another training and competency program before providing services
• Must be able to successfully pass the competency program of the care center
• Current CPR certification

The hourly pay for this role will range from $14.00 to $23.50 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
