Job summary

Grace At Work is seeking a Home Health Aide to join our team and provide one-on-one in-home care according to a patient care plan. You will work with patients directly, building genuine relationships while ensuring safety and comfort.

Requirements include completion of a Home Health Aide program, a HS diploma or GED, and a valid driver’s license with CPR certification. Prior home health experience is a plus.

Qualifications

• Graduated from an accredited Home Health Aide program.
• High School Diploma or GED.
• One year prior professional experience.
• Driver’s license required.
• CPR certification required.

Responsibilities

• Assist patients with daily activities such as moving, bathing, or personal care.
• Care for patients by cleaning, changing linens, and housekeeping as needed.
• Maintain records of patient care and report observations to supervisor.
• Engage with patients to keep them mentally healthy and attentive.
• Administer medications and immunizations as directed.
• Encourage patients with exercises or activities.

Job Summary

We are looking for a Home Health Aide to join our team! You will directly work with patients, following a one-on-one care plan in patient homes. You operate with a big heart, not only providing personalized and attentive care but by building genuine relationships with those you serve. You are detail-oriented, highly organized, and committed to creating a healthy environment that meets patient needs. Strong verbal and written communication skills are needed to succeed in this role. Prior Home Health or Hospice experience is a plus.

Responsibilities

• Assist patients with daily activities such such as moving in and out of beds, baths, wheelchairs, or automobiles
• Care for patients by changing bed linens, doing laundry, cleaning the home, or assisting with personal care
• Maintain records of patient care, condition, progress, or problems to report and discuss observations with the supervisor
• Build relationships with patients by conversing or reading aloud to help keep them mentally healthy and alert
• Give medications and immunizations
• Engage patients in exercises or other activities
