Dialysis Clinic, Inc. in Massachusetts is seeking a Home Therapies Nurse to coordinate the clinical and administrative aspects of the home dialysis program, including home hemodialysis and peritoneal dialysis options.

You will collaborate with the medical team, plan home training sessions, clinic visits and patient care conferences, and monitor supplies to ensure high quality patient care. This full-time role requires an MA RN license, a valid driver's license, and a compassionate, team-focused

Qualifications

• Accredited nursing degree and MA RN license required.
• Dialysis experience a plus.
• Current MA RN license required.
• Valid driver's license with clean driving record.

Responsabilités

• Collaborate with medical team on patient care concerns and plan care.
• Coordinate home training sessions, clinic visits and patient care conferences.
• Monitor patient supply ordering and usage with focus on quality care.
• Communicate regularly with interdisciplinary care team about home program activities.
• Perform all registered nurse functions and maintain documentation.
• Maintain records per ESRD Network, regulatory agencies and policy.
