Job summary

Imagine the Possibilities, Inc. is seeking an HCBS Programmer to support administrative operations in a Home and Community-Based Services setting. The role focuses on leading a team, coordinating services, and ensuring person-centered outcomes for Iowans with diverse needs.

You will monitor services, develop strategies, and coordinate with CBCM and IDT to meet individualized goals. This role emphasizes empowerment, collaboration with families, and compliance with applicable regulations.

  • Act as liaison to families, guardians, and case managers; communicate changes and promote community partnerships.
  • Maintain compliance with laws and policies and advocate for independence and dignity.

This role operates administratively in a Home and Community-Based Services (HCBS) Setting, defined as “medical, social, and supportive services for Iowans with functional, cognitive, and other physical or mental health needs” (Iowa Health & Human Services). Rather than institutionalizing individuals who have these physical and mental health needs, HCBS settings provide these individuals with the opportunity to live and receive services in a way honoring to them and their abilities.

As a core team member, you’ll be responsible for monitoring services, creating new strategies to reach goals, and coordinate services in a way that empowers people to meet and exceed their goals.   • Team Leadership & Culture Development: Build and maintain a strong team culture through active presence, monthly team meetings, staff coaching, on-call participation, and providing coverage as needed, including short-notice staffing gaps.
  • Resource & Operational Oversight: Manage and monitor key resources including vehicle usage, individual budgets, payee-related financial processes, medication check-ins, and appointment scheduling to ensure services run smoothly and efficiently.
  • Person-Centered Service Coordination: Ensure services are delivered in a way that respects individual needs and goals by collaborating with CBCM and IDT, developing individualized plans, completing assessments, and maintaining accurate service documentation and authorizations.
  • Communication & Community Collaboration: Serve as the primary liaison for individuals served, families, guardians, and case managers; communicate service changes promptly, document interactions, and support community partnerships and service promotion efforts.
  • Compliance, Documentation & Advocacy: Maintain compliance with all applicable laws, policies, and regulations; ensure accurate incident reporting and service documentation; and advocate for individuals to promote independence, dignity, and self-determination.

  • Driver’s License: You’ll be responsible for transportation at times, so we need to trust your ability to be safe behind the wheel.
  • Reliable Transportation: You’ll need reliable transportation. You will also need to provide proof that your automobile insurance covers $100,000/$300,000/$50,000 liability coverage – don’t worry, we can answer any questions you might have about this.
  • Educational Requirement: Minimum Education Requirement of High School Diploma/GED equivalent.

Physical Demands:

  • May require lifting at times, up to 35 pounds. Lifting more than 35 pounds requires the use of a two-person lift, mechanical lift, or assistive equipment.
  • May require prolonged sitting and working at a computer for up to eight hours.
  • May require frequent bending, stooping, and stretching.
  • May require prolonged standing for up to eight hours.
  • Must wear hearing and eye protection in required areas.
  • Must have ability to see objects/persons at a distance.
  • Must have ability to see close work such as typed or handwritten material.
  • Must have ability to hear conversations in a quiet and a noisy environment.
  • Must have ability to determine where a sound is coming from.
  • Must have ability to communicate through speech.
  • Must have ability to drive and transport for service provision as needed.
  • Must remain awake during working hours, if applicable.
