Position Purpose

As a Speech Therapist at Trinity Health Of New England At Home, you will provide professional speech and audiology diagnostic and therapeutic services to agency clients as authorized by their physician.

Key Responsibilities

• Client Assessment & Planning: Accurately assess clients at admission and required time points using standardized documentation. Develop a treatment plan in collaboration with the physician and client, focusing on achievable goals to improve or stabilize the client's condition.
• Visit Prioritization: Effectively prioritize and plan visits based on client capabilities, condition, physician orders, and available resources.
• Regulatory Compliance: Maintain a working knowledge of Federal, State, and local laws and regulations, including the Organizational Integrity Program, Code of Ethics, and agency policies.
