Caring Angels Home Care, LLC in Manhattan is seeking a part-time or full-time Speech Language Pathologist for our pediatric home-based caseload. The role offers a base salary of $125,000 annually plus a comprehensive benefits package and door-to-door car service.

Responsibilities include evaluating patients, conducting tests, developing plans of care, documenting progress, coordinating with nurses, and reporting outcomes.

Qualifications

• Currently licensed in New York State as a Speech Language Pathologist.
• ASHA CCC-SLP certification required.

Responsabilités

• Evaluate speech and language rehabilitation needs and provide services as ordered.
• Perform clinical tests to assess disorders and develop care plans with goals.
• Provide SLP procedures to achieve identified goals and educate patients/families.
• Maintain patient records with treatment details, progress, and discharge summaries.
• Collaborate with health team and nurse to coordinate care and monitor progress.
• Report initial findings and plan, within 24 hours of first visit.
• Document visit notes and findings in designated forms as required.
• Communicate weekly with the nurse to discuss progress.
• Re-evaluate patients as needed and generate progress reports.

Caring Angels Home Care is looking for a part-time and full-time dynamic Speech Language Pathologist for our Home based pediatric cases in Manhattan. This position offers a base salary of $125,000.00 plus an excellent benefits package which includes: Medical and Dental insurance, 401k Plan after only 3 Months of Employment, 27 Paid Time Off Days, 10 Holidays, FFS available at $100.00 rate per 30 minute Session. The Option to cover Caseload on Weekends is also available.

Our part-time and full-time positions offer therapists the option to have a personal driver pick you up from your home and drive you to your cases and then back home. Currently, we are the only agency in New York that offers this unique door-to-door car service!

Speech Language Pathologist Job Description

1. Evaluates the Speech and Language Rehabilitation needs of the Patients and provides Speech and Language Rehabilitative Services for Patients as ordered by the Patient’s Authorized Medical Practitioner.
2. Performs clinical tests to assess Speech and Language disorders and potential for restoration. Develops a Plan of Care with restorative and/or maintenance goals to meet identified needs.
3. Performs Speech Language Pathology procedures to achieve identified goals.
4. Instructs Patients/Family and other appropriate persons/health team personnel in the use of assistive modalities and appropriate Speech and Language Procedures.
5. Maintains records for each Patient treated by documenting general information, special precautions, description of and number of treatments rendered, reaction to treatment, progress or lack of progress made and Discharge Summary.
6. Works with health team to shorten the Patient’s recovery from illness and the management of residual disability.
7. Makes telephone contact, followed by a written report with the Nurse to report initial findings, goals, visit frequency and estimated length of service within 24 hours of initial visit.
8. Charts all visit contacts and findings on designated forms, within twenty-four (24) hours, and as required by agency policy.
9. Communicates with the Nurse at least every two (2) weeks to discuss the Patients progress.
10. Re-evaluates Patients as needed to detect progress attained and completes a written progress report for the Patient’s record containing Patient’s progress, continued treatment plan, goals, frequency of visits and outcomes of care provided.
11. Performs other related duties assigned by supervisor. In an emergency, performs other duties necessary for the welfare of Patients or the efficient operations of the Agency.

License/Registration/Certification/Permit Required:

• Currently licensed in New York State as a Speech Language Pathologist. Current certification of clinical completion in Speech from the American Speech and Hearing Association.
