Job summary

Gurwin Healthcare System seeks a Social Worker for its Home Care Division to deliver compassionate social work services in patients' homes across Nassau County. This per diem role emphasizes collaboration with nurses and community services to ensure comprehensive care and entitlement support.

Applicants should have an MSW, NYS social work certification, and at least one year of healthcare social work experience within a multidisciplinary team. Travel to patient homes is required.

Qualifications

• Master's Degree in Social Work from CSWE-accredited program.
• NY State certification to practice Social Work preferred.
• Minimum 1 year of social work experience in health care as part of a multidisciplinary team.

Responsibilities

• Assess client medical/home situation and financial resources.
• Collaborate with Community Health Nurses to develop treatment plan.
• Provide direct social work services and community referrals as appropriate.
• Assist patients in obtaining entitlements and addressing social/emotional barriers to treatment.
• Prepare progress notes and ensure records are properly filed and shared with relevant agencies.
